Transdiagnostic study of Adverse Childhood Experiences in people with mental health problems (TrACE)
Adverse childhood experiences (ACEs), such as maltreatment, are a significant risk factor for severe mental health problems, often reported by psychiatric inpatients. These individuals often experience complex psychopathology, functional impairment, and chronic conditions. Mental disorders with high ACE prevalence, like borderline personality disorder (BPD), persistent depressive disorder (PDD) or complex PTSD, commonly involve difficulties in social interactions and increased loneliness, which may hinder the effectiveness of psychotherapy. Rejection Sensitivity, the tendency to anxiously expect and react to rejection, often stems from ACEs, including experiences of parental rejection and various forms of abuse (emotional, physical, sexual), and worsens mental health outcomes. This study explores the link between ACEs, attachment and interpersonal problems, and the effects of therapies like CBASP and DBT, while also examining potential biological predictors, such as changes in the oxytocin system.
- Naturalistic study to transdiagnostically assess the influence of adverse childhood experiences on interpersonal difficulties (e.g. rejection sensitivity and loneliness) on behavioral, structural, and neuroendocrinological level
- Cross-sectional arm evaluating a new transdiagnostic and dimensional construct using psychophysiological and neuroendocrine measures, eye-tracking and MRI-scan
- Two single naturalistic psychotherapy arms (CBASP and DBT)
- Funded by LMU Klinikum and the Deutsche Forschungsgemeinschaft (DFG), grant to AJ

AMACE: Analysis and Measurement of Adverse Childhood Experiences Project
Despite the well-established relevance of Adverse Childhood Experiences (ACEs) in mental health research, there is significant heterogeneity in their conceptualization, measurement, and analytical strategies. Existing ACE scales and interviews exhibit inconsistencies in the constructs they assess and, in the ways, adverse experiences are quantified—such as frequency, emotional impact and severity, and timing. Statistical analyses commonly rely on dichotomous indicators, total scores, or subscale scores, which may oversimplify the complexity of ACEs. This heterogeneity presents challenges to establishing a unified understanding of ACEs and their impact on mental health, underscoring the need for robust conceptual frameworks and methodological rigor in future research. The objectives of the AMACE project are (1) To systematically review available instruments for measuring ACEs, evaluate their conceptual overlap, and identify gaps in assessed experiences. (2) To develop advanced statistical strategies that account for the co-occurrence and intertwined effects of different ACE types.
- Methodological project to describe the status quo of ACE assessment and analysis
- Identification of clusters of co-occurring ACEs within mental health research
- Characterization of ACE clusters in relation to environmental, social, and neurobiological determinants.
- Examination of ACEs as risk factors for the development of adverse clinical outcomes.
- Evaluation of differential treatment effects in patients with varying patterns and severity of ACEs.
CHANGE-PDD: Cognitive Behavioral Analysis System of Psychotherapy (CBASP) vs. Behavioral Activation (BA) in persistently depressed treatment-resistant inpatients: Efficacy, moderators, and mediators of change
A large multicenter randomized controlled trial of the University of Greifswald (PI: Eva-Lotta Brakemeier) is currently conducting the ChangePDD study. Its aim is to compare the effectiveness of psychotherapy programs for individuals with PDDs who have not sufficiently responded to other therapy attempts so far (Cognitive-Behavioral Analysis System of Psychotherapy vs. Behavioral Activation). We are participating in this study on two psychotherapeutic wards.
- Randomized multicenter trial to compare CBASP conducted over 16 weeks (acute and continuation treatment) with BA (same dose and duration) in PDD inpatients with treatment resistance regarding efficacy, moderators and mediators of change
- Funded by Deutsche Forschungsgemeinschaft (DFG)
- Clinical Trials Register: NCT04996433

MAAP: Mechanismus-basierte Anti-Aggression Psychotherapie
Up to 80% of individuals with borderline personality disorder (BPD) exhibit aggressive behaviors over the course of a year, and more than 50% are occasionally or frequently involved in physical altercations. Despite the significant burden placed on those affected and their social environment, there are currently no specific, cost-effective treatment options for aggressive behavior. To address this research and treatment gap, a manualized short-term anti-aggression psychotherapy (MAAP) for group settings was developed in Heidelberg by Sabine C. Herpertz and her team. This therapy incorporates validated techniques from Dialectical Behavior Therapy (DBT) and Mentalization-Based Therapy (MBT), specifically targeting outward-directed aggression in individuals with BPD.
- Randomized-controlled-trial comparing effects of a group intervention on aggression with supportive psychotherapy
- Funded by the Deutsche Forschungsgemeinschaft (DFG)

SMART-SI: Smartphone-based phenotyping of suicidality and interpersonal risk factors
Suicide remains a leading cause for mortality among psychiatric disorders. Our research has shown a link between suicidality in chronically depressed patients and feelings of loneliness and isolation. However, underlying temporal dynamics remain unclear. Smartphone-based Ecological Momentary Assessment (EMA) enables dynamic tracking of these aspects. Building on previous work, the SMART-SI study uses EMA to explore the dynamic relationship between interpersonal risk factors, such as loneliness, and suicidality. The goal is to enable targeted just-in-time interventions (Ecological Momentary Interventions (EMI)) for suicide prevention.

Overcoming Loneliness: Development and evaluation of a mechanism-based psychotherapeutic intervention.
Loneliness is a major health concern and contributes to the symptom burden and persistence of psychiatric disorders. However, effective loneliness interventions are sparse as loneliness constitutes a highly individual and complex emotional experience. Despite its complex nature research has identified several mechanisms that maintain loneliness, such as increased rejection sensitivity, an impaired social network and a lack of self-compassion. Therefore, specific individualized intra- and interpersonal strategies that target these mechanisms may be promising in alleviating loneliness.
Aim of this project is the development and evaluation of a newly designed group intervention to tackle loneliness. Besides the feasibility, effects of the intervention on key mechanisms are investigated in order to gain a deeper understanding of loneliness.
- Randomized trial comparing the effects of a group intervention on loneliness to treatment as usual
- Group intervention designed for inpatient and outpatient samples
- Funded by the MCSP of the Medical Faculty of the LMU, grant to MR
Well-Being and Recovery: Exploring Well-Being Therapy (WBT) as Group Therapy in a Day-Clinic setting
Well-Being Therapy (WBT) by Fava shifts the focus of psychotherapy from reducing suffering to enhancing positive feelings and achieving balanced well-being, addressing an often-overlooked aspect of mental health. This approach can significantly improve quality of life, particularly for individuals with chronic mental illnesses or residual symptoms. By fostering self-observation and working on dimensions of well-being, WBT strengthens mental resources, stability, and resilience, enriching the psychotherapy landscape. Combining behavioral therapy with growth-oriented elements like flow experiences and lifestyle changes, its transdiagnostic nature makes it well-suited for psychiatric day clinics and patients with different mental disorders.
- Single-arm naturalistic trial evaluating acceptance, feasibility and effectiveness of WBT as group therapy in our day-clinic setting
